A patient/his or her next-of- kin/agent/lawyer can obtain medical report from Unit Medico Legal, Jabatan Rekod Perubatan, Wisma Rekod (opposite Maternity Hospital) or send it an application via mail.
Patient or his/her next–of-kin/agent/lawyer must bring along:-
- Application Form/Letter of Application. (PER ML-102)
- Copy of patient’s clinic appointments card/ hospital bill/ dicharge note.
- Copy of patient's Identification Card
- Insurance / KWSP / PERKESO form (If require)
- Consent letter given by patient (required for agent, his/her next-of-kin).
Expected duration of completion for a report is within 8 week(2 months).
