Surgery is one of the basic services offered by the hospital since the beginning and before the 60's it incorporated Urology, Neurosurgery and Plastic and Reconstructive surgery. The first Malaysian general surgeon to head the department was Dato' Dr. Syed Mohammad Alhady; prior to his appointment all general surgeons were expatriates as were all other consultant specialists.

In the 60's when surgeons started to specialize, separate departments for Urology, Plastic and Reconstructive Surgery and Neurosurgery were set up. Thoracic and close Cardiac Surgery were offered at the Lady Templer Hospital till the 70's but by the early 1980's a new department of Cardiac Surgery was set up which became the 'forerunner' of The National Heart Institute.

Paediatric Surgery became a separate unit in the 70's and is now housed separately in the Paediatric Institute. Hepatopancreaticobiliary Surgery has been moved to Selayang Hospital since September 2000. Breast and Endocrine surgery has extended a unit to Putra Jaya Hospital by mid-2002.

At present the services offered by the department are confined to Vascular, Breast and Endocrine with General Gastrointestinal and Colorectal surgery. Upper and and lower endoscopy forms part of General surgical services.
